COVID-19 RESPONSE FRAMEWORK: KEEPING ONTARIO SAFE AND OPEN

Please be advised that customers and patrons will be asked to screen before entering our business premises in adherence to Public Health Official Guidelines and as such will need the following COVID-19 Questionnaire completed and submitted to us in order to enter our store prior to Requesting to Book an Appointment, as well as arrive for your appointment once your booking has been confirmed thereafter.

I do not pose a concern for a potential COVID-19 infection (e.g. there was an outbreak that I may have been exposed to, or waiting on COVID-19 test results etc.)

I haven’t travelled or been in close contact with someone who has or is suspected to have Covid-19 in the last 14 days

Despite procedures that FAH HAIR EXTENSIONS implements to disinfect the Studio, I understand that there is a risk of exposure to COVID-19 and I will not hold FAH HAIR EXTENSIONS liable should I be exposed to COVID-19. I understand it is my sole responsibility to seek medical attention at my own expense
